Is it assumed that “et. al” is always written as such and in italics
or is this defined somewhere? I can’t find it in the apa.csl nor in
the csl.rnc.

I think it dropped out of the version, but should be configurable.
Let’s resolve the other issues, and then I’ll add it back.
