capitalizing family names

Another wrinkle we don’t cover is where author family names need to be
capitalized.

I’d like to propose adding an attribute to cs:names to cover this:

 attribute givenname-format { "capitalize" }

The alternative would be too ugly to be worthwhile in my view.
