Does the subsequent-author-substitute apply to all authors, or only
the first author? Do all authors have to match or only the first one?

This situation:

An “author” for these purposes represents the group of authors. Where
the group changes in ANY way (including order), it’s a different
"author."

Same logic applies to generate the year suffix of course. So:
